Insurance & fees

I accept the following insurance:

  • Moda

  • Kaiser

  • Pacific Source

  • Blue Cross/Blue Shield

  • Regence

  • United Health Care/Nike & Intel

  • Single case agreement OHP

You are responsible for all fees not paid by your health insurance. I recommend that you contact your insurance company and ask the following:

  • Is Catherine Jones LPC, covered as a preferred provider or are her services out of network? Please note that I'm not in network with UHC, UBH, or Providence Health Plans.

  • What is my annual deductible, and have I met it? Is my deductible waived or do I have to meet the deductible before my plan pays for counseling?

  • What is my co-pay or co-insurance for each session?

  • If Catherine Jones LPC is out of network, what percentage of her services does my plan cover? What percentage is my responsibility?

  • Are there limits on the number of outpatient mental health visits covered?

  • Will my plan cover telehealth services?

Private Pay/Out of Pock Cost:

Intake appointment and evaluation is $225, ongoing individual sessions $185-$215. Accept cash, check, credit card, HSA/FSA debit cards at the time of service. Limited sliding scale available.

To use insurance, it is necessary to meet the criteria for a mental health diagnosis. This diagnosis is reported to your health insurance and become a part of your medical record. Private Pay/Self Pay is an option if you do not have health insurance or if you do not want to use your insurance for counseling.

No show or late cancel fee:

Please allow 24 hours to cancel or a $65 dollar cancellation fee will be charged. Fees for therapy are due at the time of services. I accept all major credit cards, check, cash and HSA/FSA debit cards.

No Surprises Law - Disclosure Notice:

You have the right to receive a "Good Faith Estimate" explaining how much your medical are will cost Under the law, health care providers need to give patients who don't have insurance or who are not suing insurance an estimate of the bill for medical items and services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related cost like medical tests, prescription drugs, equipment, and hospital fees.

Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical services or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
